We are looking for a highly motivated individual to work with us in out Adult Social Work Team, East Lothian Health and Social Care Partnership.

We are currently seeking a dedicated and compassionate Community Care Worker to join our dynamic Adult Social Work Team. This is an exciting opportunity to be part of a team that is consistently recognised for its high performance and commitment to reducing waiting times and improving outcomes for vulnerable adults.
Our team provides support to adults across a wide spectrum of needs, including:
1. Older adults
2. Individuals with acute and enduring mental health conditions
3. People living with physical disabilities
4. Adults with autism and neurodiverse needs
As a Community Care Worker, you will play a vital role in:
5. Holistic assessment and person-centred care planning
6. Case management and regular reviews
7. Participating in our fast-paced duty system to respond to urgent needs
