About the Role
We are seeking an experienced Senior Locum Social Worker to join a busy Hospital Discharge Team, playing a vital role in supporting safe and timely discharges through the Discharge to Assess (D2A) process. You will work closely with multidisciplinary teams to ensure individuals are discharged to the most appropriate setting, with ongoing assessment and support provided in the community.
Key Responsibilities
* Support safe and timely hospital discharges in line with D2A pathways (1, 2, and 3) and local discharge policies
* Complete Care Act assessments, Mental Capacity Assessments, and Best Interests decisions
* Develop strengths-based, person-centred discharge and care plans
* Coordinate community support, reablement services, and short-term placements
* Work collaboratively with MDT colleagues, patients, and families
* Identify and respond to safeguarding concerns in line with statutory responsibilities
* Maintain accurate and timely case records within a fast-paced environment
* Provide supervision to a team member, ensuring high standards of practice and performance
* Participate in the Duty Manager rota, including triaging referrals and allocating work
