Overview
We are recruiting an IBD Clinical Nurse Specialist to provide expert care for our Inflammatory Bowel Disease patients. The role requires an enthusiastic nurse with a sound vision of nursing and a commitment to deliver best patient care. Knowledge of IBD and its symptoms is desirable but not essential. The post holder will work under the supervision of the Lead IBD CNS and Band 7 IBD Clinical Nurse Specialists.
The role involves supporting patients face‑to‑face and over the telephone, working with multidisciplinary teams, and ensuring best practice in care delivery across outpatient and inpatient settings.
Responsibilities
* Provide support and advice to IBD patients face‑to‑face and over the telephone.
* Work with the existing team to deliver continuity of care across outpatient and inpatient settings.
* Collaborate with primary care teams and communicate within the multidisciplinary team.
* Lead IBD drug start‑up and outcome monitoring clinics.
* Attend IBD multidisciplinary meetings.
* Report on multidisciplinary clinical management and patient outcomes.
* Refer patients for multidisciplinary case‑management discussions.
* Perform direct care in the infusion clinic.
* Provide IBD patient education.
* Manage IBD telephone and email consultations.
* Ensure appropriate blood and treatment monitoring.
* Manage IBD home‑care clinical monitoring, prescription management and ICB reporting.
* Support professional activity and participate in professional development and CPD.
