Job Responsibilities
* Carry out medical services with due care and attention, using skills and experience in accordance with good medical practice.
* Triage patients over the phone and make a clinical decision about appropriate care.
* Investigate and diagnose with a care plan or management to follow.
* Manage medicines and occasional urgent tests.
* Conduct home visits and face‑to‑face urgent care centre appointments.
* Work closely with operations and clinical staff.
* Work with and support all clinical colleagues and operational teams.
* Work with clinical leads and the medical director to challenge and improve the service.
* Ensure all clinical reports are kept to the highest standard.
* Consider the needs of the community and the wider determinants of health.
* Maintain accurate clinical records.
* Collaborate with other health professionals and providers to bring optimal expertise around the individual patient, including secondary care and community NHS services.
* Integrate social prescribing into patient care.
Alder Hey: Onsite
* Assess, diagnose and provide advice, care and management to children and young people presenting with undiagnosed or undifferentiated illness via face‑to‑face assessments, within our paediatric primary care streaming.
* Demonstrate clinical reasoning, critical thinking and autonomous practice supported by APNP colleagues in the Urgent Care Centre at Alder Hey Children’s Hospital.
* Provide high‑quality holistic clinical care aligned with current healthcare standards.
* Develop a shared decision‑making approach where appropriate.
* Collaborate with other health professionals and providers to bring optimal expertise around the individual patient.
* Integrate social prescribing into patient care.
* Work with clinical leads and the medical director to challenge and improve the service.
Salary and Working Conditions
Salary: £108,664.00-£130,398.00 per annum (pro‑rata) or £76.43-£91.48 per hour for bank employees.
Department: Out of Hours, Primary Care & Alder Hey
Location: Various locations within Merseyside, Sefton & Halton, including Wavertree Head Office and The Primary Care Hub.
Shifts: Full time/part time and bank positions available (depending on department).
Qualifications
* Full GMC Registration and licence to practice.
* Up to date Enhanced DBS Certificate (we will apply for one if you are not a member of the DBS Update Service).
* Understanding of clinical risk management and clinical governance.
* Ability to take independent clinical decisions when necessary and seek further advice from colleagues as appropriate.
* Ability to manage own time and workload in line with SLA.
* Excellent interpersonal skills and ability to communicate sensitively.
* Demonstrate skills in written and spoken English adequate to enable effective communication.
* Competent in the use of IT systems including ADASTRA and EMIS.
