We have an exciting opportunity for an enthusiastic and motivated Band 4 Diabetes Associate Practitioner to join our friendly and supportive diabetes service in Swale. Working under the supervision of registered clinicians, you will play an important role in delivering structured, high‑quality diabetes care across community and inpatient pathways. You will support patients living with diabetes to better understand and manage their condition, helping to improve outcomes and reduce complications. This role is ideal for someone with experience in health or social care who has a strong interest in diabetes, health promotion and long‑term condition management, and who is ready to develop their clinical skills within a specialist service. You will work closely with nurses, dietitians and other healthcare professionals as part of a multidisciplinary team, contributing to patient‑centred, evidence‑based care.

Main Responsibilities

As a Band 4 Diabetes Associate Practitioner, you will support the delivery of structured diabetes care to adults with long‑term conditions. This will include undertaking agreed clinical tasks, monitoring patients’ health and wellbeing, and supporting registered clinicians in the ongoing management of diabetes care. You will work with patients to promote self‑management, providing education and practical support around lifestyle, medication adherence, blood glucose monitoring and recognising signs of deterioration. You will contribute to clinics, reviews and follow‑up appointments, ensuring patients feel informed, supported and involved in their care.

You will be responsible for maintaining accurate clinical records, communicating effectively with the wider multidisciplinary team, and escalating concerns appropriately. You will also contribute to service improvement, audit activity and the ongoing development of diabetes pathways within Swale.


Person Specification

* Experience working in a health or social care setting with direct patient contact
* A strong interest in diabetes care and supporting people with long‑term conditions
* Confidence undertaking delegated clinical tasks under the supervision of a registered clinician
* Good communication skills and the ability to engage with patients, carers and colleagues
* Ability to work effectively as part of a multidisciplinary team
* Basic IT skills and confidence using electronic patient record systems
* A flexible, organised and professional approach to work
* Previous experience working with patients with diabetes or other long‑term conditions
* A relevant Level 4 qualification or working towards one
* Experience in community or outpatient settings
* An interest in health promotion, education and prevention
