Nurse Practitioner

The Nurse Practitioner is an integral part of the clinical shift. They are essentialsource of knowledge, support, experience and skill to other staff within thedepartment. The Nurse Practitionerwill maintain clinical safety and will see approximately19 patients a shift; the department currently sees an average of 154 patient sa day requiring 12 clinical staff (6 in the morning/ 6 in the afternoon).

The post holder will work 30 hours per week over 3 x 10 hour shifts shifts.


Main duties of the job

* To act as part of Urgent Treatment centre team, seeing patients with undifferentiated and undiagnosed health problems; liaising with others to maintain and enhance a culture which promotes effective multidisciplinary team working within the organisation and across the wider health economy.
* To provide care and advice to clients utilizing evidence based practice to promote effective treatment and health promotion.
* Assess, plan and implement treatment plans using evidence based nursing care
* Acts as an autonomous practitioner within agreed protocols guided by organisational policies and within scope of professional practice.
* Works under indirect supervision in the provision of care to clients, accessing the knowledge and skills of the team leader as required.
* Act as a mentor to pre and post registration nurses, allied health professional and unqualified students.
* The post holder should demonstrate flexibility and be able to work across a range of shift patterns.
* Participates in the education, development and line management of other staff members
* Is an effective team player within the interdependent unit, operating flexibly across both units as required and recognising the benefits of the integrated approach

Job responsibilities

* Maintain a high standard of patient care in accordance with NMC Code of Professional Conduct and within Scope of Practice.
* Maintains own clinical and professional expertise by participating in clinical supervision, professional development planning and within NMC Code of conduct and Scope of professional practice.
* Act as a resource for other members of the Urgent Treatment centre team
* Promote the clinical effectiveness of the nurse-led service using a multidisciplinary team approach.
* Contribute to the development of the Urgent care centre service.
* Promote a culture which utilizes latest evidence and best practice.
* Works autonomously under indirect supervision liaising with team leaders and co-ordinators as required.
* Work in a flexible way to support and maintain good team working practices.
* Utilizes analytical and judgemental skills in the assessment of patients presenting with undifferentiated and undiagnosed health problems.
* Demonstrate commitment to multidisciplinary team working across the local health economy.
* As a member of the Urgent Treatment centre team contributes ideas for service development to the management team.
* Contribute to the development implementation and monitoring of policies, procedures and clinical guidelines as appropriate.
* Participate in the development of an effective, nurse-led service utilizing a holistic, health needs approach.
* Provide specialist clinical knowledge and advice to clinical colleagues, patients, carers/relatives.
* Supervise new and junior staff i.e. health care assistants.
* Participate and promote the development of nursing professional practice within the framework of the Modernisation agenda.
* Adopt a positive approach to change.
* Act a role model for junior and unqualified staff.
* Act as a mentor to pre and post registration students, allied health professionals and unqualified students.
* Participate in and promote the development of a multidisciplinary approach to health care provision.
* Exercise a high degree of clinical autonomy and professional judgement, treating independently or where necessary referral to other health care providers under the indirect supervision, and following discussion with the team leader.
* Receive patients with undifferentiated and undiagnosed health care problems appropriate to level of competence and scope of practice.
* Possess skills in the management of a range of minor illness and injuries and chronic disease management.
* Undertake a range of advanced clinical procedures, under supervision, until deemed competent.
* Provide health promotion and education, screen patients for risk factors and early evidence of illness.
* Supply and administer drugs, treatments and appliances under PGD as appropriate.
* Initiate clinical tests and examinations as appropriate following discussion with team leader.
* Offer a complementary service to that of other health care providers within the local health economy.
* Act as an advocate for the patient at all times.
* Demonstrate a clear understanding of the legal and ethical issues with regard to nurse-led services and professional accountability and responsibility.

* Maintain own clinical and professional expertise by participation in clinical supervision, personal development planning and within the scope of professional practice and NMC Code of conduct.
* Provide supervision and mentoring opportunities for junior Urgent treatment Centre staff and students as appropriate.
* Educate other members of staff utilizing formal and informal methods as appropriate.
* Participate in clinical audit as required.
* Promote a research culture and actively encourage the utilization of evidence, research and audit to develop practice.
* Utilize evidence to provide effective and appropriate care to clients.
* Maintain adequate and comprehensive documentation in accordance with NMC and local requirements of standards for record keeping.
* Promote and maintain effective working relationships across the local health and social care community, service users and voluntary sectors.
* Promote the development of clear systems of communication and referral both within the Urgent Treatment Centre and with other health care providers.
* Maintain confidentiality as per local, national and NMC guidance.
* Adapt personal communication skills to overcome barriers to communication and understanding.
* Demonstrate profession skill in facilitating effective and clear communication in dealings with sensitive or volatile situations i.e. child protection issues.
* Utilize a variety of systems to communicate clinical information in accordance with the data protection act (fax, telephone, letter and email).
* Utilize personal skills to facilitate effective and clear communication with clients to ensure collaborative health care and concordance with recommended advice and treatment.

Job Specific Requirements

Qualifications

* Registered first level nurse/ Qualified Paramedic Practitioner
* Current NMC/HCPC registration
* Evidence of teaching of students
* Degree or able to demonstrate evidence of study at level 6
* Independent prescriber holding (non medical prescribing V300)
* Clinical Examination module or equivalent at level 6 or 7
