Job Purpose
Monday - Sunday, 6 or 8.30 – 4.30
To provide skilled nursing care working under the supervision of the Community Nurse Team Lead and Deputy Team Lead to patients in a variety of community settings.
Main Duties and Responsibilities
1. Provide skilled nursing care based on best practice and clinical evidence, including chronic disease management, end of life care, continence promotion, and wound management. Skills include phlebotomy, medication administration via syringe drivers and intravenous therapy, PEG management, urinary catheterisation, and tracheostomy suctioning.
2. Act as a Named Nurse with support from the DN Team Lead, responsible for holistic health needs assessment, care planning, evaluation, and ensuring continuity of care following referrals.
3. Undertake comprehensive assessments of patients' physical, psychological, and social needs using the Single Assessment Process, including carers' needs. Develop individualised, person-centred care plans promoting self-care to prevent hospital admissions.
4. Act as the patient’s advocate, facilitating their choices regarding nursing care, and promoting independence and self-care as appropriate.
5. Identify and record new problems and relevant information, reporting to the DN Team Lead as needed.

Additional Responsibilities
* Participate in implementing and monitoring the Trust’s clinical and operational guidelines and policies.
* Act in accordance with the Trust’s Organisational and Clinical Guidelines and Policies, as well as the NMC Code of Conduct and other professional guidelines.
* Ensure safe handling of body fluids and contaminated sharps.
* Be alert to the needs of vulnerable adults, including risk assessment and appropriate action.
* Develop and utilise specialist nursing skills such as leg ulcer assessment, syringe driver setup, intravenous drug administration, continence assessment, and palliative care.
* Support patients and carers with medication management and reviews.
* Provide care to people with long-term conditions, undertaking reviews as per individual care plans, with support from experienced colleagues.
