Overview
The Adult Critical Care Transfer Service (ACCOTS)–the dedicated, commissioned Adult Critical Care Transfer service for the Midlands Region–is hosted by two acute Trusts: University Hospital of Leicester (East Midlands) and University Hospitals of Birmingham (West Midlands). ACCOTS provides consultant-led triage, decision support and co-ordination of referrals and patients who require or are likely to require specialist critical care escort, intensive monitoring, organ support and/or specialist treatment. We undertake transfers for escalation of care (including time-critical and urgent), repatriation, continuation of care and capacity reasons. Our mission is: “To deliver the highest standards of compassionate critical care to patients undergoing transfer within and from the Midlands Region.” ACCOTS aims to be responsive, caring and to maintain high standards while looking after our people.
This is an exciting opportunity to join the ACCOTS team at the East Midlands Base in Castle Donington. We are looking to build a substantive medical team of consultants with a minimum 2.0 PA contract (in extenuating circumstances, we may consider less). SPA will be paid pro-rata. ACCOTS in the East Midlands is based at Trent Lane Industrial Estate in Castle Donington. The successful applicant will work with the Lead Transfer Nurse and Transfer Practitioners to deliver high-quality critical care transfers as a Duty Consultant.
Role and Responsibilities
* Triage, Co-ordination and decision support for all referrals to ACCOTS.
* Supervision and delivery of clinical care.
* Teaching, training and continued professional development.
* Governance and quality improvement.
* Demonstrate exemplary leadership behaviour that supports ACCOTS, values and strategy.
* Ensure the safe, timely and efficient transfer of critically ill patients within the ACCOTS operating region.
* Work with the Lead Transfer Nurse and Transfer Practitioners to deliver high-quality transfers and manage transfer resources.
* Maintain high-quality clinical records and participate in induction and ongoing training related to referral handling, triage and decision support.

This role reports to the Clinical Lead and Clinical Director, and to the ACCOTS General Manager.

Background and Context
Adult Critical Care Transfer Services (ACCTS) are commissioned by NHS England to provide regional coordination, triage, decision support and transfer care for critically ill patients requiring inter-hospital transfer. The service aligns with NHS England’s Adult Critical Care service specification and standard critical care practice within hospital settings. Patients require critical care transfer for escalation, repatriation, continuation of care, or capacity reasons.
Role Purpose and Main Duties
* Triage, coordination and decision support for referrals; prioritise and maximise resource use across referring and receiving hospitals and clinical teams.
* Provide or supervise clinical care ensuring resuscitation, stabilization and continuation of critical care during transfers.
* Lead teaching, training and professional development; support induction and ongoing development of clinical staff.
* Governance and quality improvement: maintain knowledge of SOPs and policies; contribute to ongoing development and improvement processes; attend governance and quality days; support audit and research.
* Leadership: act as senior clinical leader per shift, model service values and behaviours; potential for additional leadership roles within the service.
Person Specification
Essential
* Full GMC registration with license to practise.
* Specialist register in Intensive Care Medicine or Anaesthesia.
* Demonstrable training and currency in delivery of in-hospital critical care.
Desirable
* Higher qualification in retrieval/transfer medicine (e.g., Diploma in Retrieval and Transfer Medicine).
* Postgraduate leadership and management qualification.
* Relevant specialist courses (e.g., Advanced Life Support, European Trauma Course, point-of-care ultrasound, difficult airway management).
Knowledge & Experience
* Compliance with GMC Good Medical Practice, appraisal and revalidation requirements.
* NHS Consultant or equivalent; experience in Intensive Care Medicine or Anaesthesia with an interest in Intensive Care Medicine.
* Experience in critical care transfer/retrieval medicine and leadership/management experience.
Skills & Abilities
* Excellent verbal and written communication and non-technical skills.
* Leadership in a high-performing small team; flexible and adaptable; able to work under pressure.
* Strong problem-solving, decision-making, time management and organisational skills.
* Evidence of non-technical and advanced communication training.
Additional requirements
* Fitness for critical care transfer work and ability to operate in ambulance environments with confined spaces.
