Overview

Are you looking for a role that provides support to patients in the community who are affected by Cancer? This may be the role for you! We are looking to recruit a Supportive Care Coordinator to be part of our MacMillan Team in Trafford. You will have the pleasure of working with patients and their carers/families throughout their journey by organising, planning and offering guidance and support in a wide range of areas.


Responsibilities

* Undertake holistic assessment and together with clients plan a programme of interventions according to their identified need, whilst utilising clinical skills and knowledge of supportive and palliative treatments; training of volunteers, students, and other staff.
* Organise and facilitate group work for the benefit of this target group of patients and carers.
* Work with the Centre Manager to provide an accessible and integrated cancer information service.
* Maintain accurate and up-to-date records on all clients to ensure documentation meets agreed standards.
