Job summary
We are on an exciting journey to test and develop a pioneering 24/7 Community Mental Health Neighbourhood Centre in the West of the City of York. This initiative has been launched, funded by NHS England for a two-year pilot, is driven by a longstanding ambition to transform mental health services in York. It builds on the collaborative efforts of the Mental Health Partnership and insights from stakeholder events, which have highlighted the need for a person-centred, transformative approach to improve outcomes for people with mental health needs.
The post holder will work with Mental Health Clinicians, Peer Support Workers, Recovery Workers, Social Workers, Social Prescribers and Care Support tively they will deliver first contact appointments to patients attending the mental health hub. The role will give you significant autonomy whilst being well supported by both the multi-agency team and TEWV.
The centre has been designed to be an open and accessible space for adults seeking mental health support, operating 24/7 with an open-door policy. Roles will undertake some weekend and out of office hours practice. Night shifts are not a function of this role. Its overarching principles are rooted in co-production, emphasising collaboration between the team and the community.
Main duties of the job
These are new, exciting and innovative posts to support the needs of the local population living with a range of mental health difficulties. The post holder will be effective members of the multi-disciplinary team. They will be professionally accountable and responsible for clinical care, providing advice, undertaking initial assessment with signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in the Hub, GP surgeries, patients homes and other primary care settings. This will include providing clinical advice to the Hub team such as specialist opinion.
The post holder will manage their daily workload and act as a specialist advisor to the team, fulfilling a pathway management function and providing short term interventions and support, in collaboration with the hub team.
You will develop links and foster a culture of partnership between Primary Care Teams, the Voluntary and Community Services and Secondary Mental Health Services, promoting clear communication and access to specialist knowledge and advice.
The role will give you significant opportunity to develop your leadership skills becoming involved in Adult Community Mental Health Transformation. You will be involved in teaching, training, and upskilling the wider system partners.

The 24/7 Community Mental Health Neighbourhood Centre in the West of the City of York has been developed as part of a longstanding ambition by the Mental Health partnership, in response to wider stakeholder events that identified that a person centred, transformative approach was needed to improve the outcomes of people with mental health needs in the city of York.
We are looking for an exceptional individual to be involved in this exciting and dynamic process to further develop the Hubs in York. The successful individual will be required to work within a multi-agency hub team and inform the development and integration of services within the Hub. The Hub team will consist of members of clinical staff, alongside a range of non-clinical staff including peer support workers and carers, community connectors and volunteers.
You will provide clinical advice, consultation, and support to other care providers within primary, secondary, voluntary and statutory services as required.
Post qualifying experience including sign off of appropriate preceptorship or competency framework is essential. Nursing candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.
Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

1. Current professional registration with an approved professional body.
2. Evidence of Continuing Professional Development.
3. For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
