What you will do:
* Assess the needs of vulnerable adults and their carers, identifying innovative and personalised support methods.
* Engage with individuals having complex needs across various settings, including the Community Team for Older People and Adults with Physical or Sensory Disabilities or Hospital.
* Manage your own caseload independently, maintaining detailed case records while working within a supportive framework.
* Implement safeguarding policies effectively, ensuring the safety and well‑being of adults in your care.
* Stay up to date with statutory legislation and guidance, applying this knowledge in everyday practice.
* Contribute to service development and take part in project management.
What you need:
* A qualification in social work (Dipsw, MSc, CQSW, or CSS) and HCPC registration.
* Substantial experience as a qualified social worker with a strong command of Adult Social Care eligibility criteria.
* Excellent assessment skills and a deep understanding of adult safeguarding legislation.
* The ability to manage a dynamic caseload efficiently, with strong evidence‑based recording skills.
* Exceptional communication abilities and conflict resolution skills.
* A knack for working independently, demonstrating self‑motivation and effective organisational skills.
* Experience in a supervisory role and proficient IT skills.
