Exeter South is a multidisciplinary team working in the community. The team includes a team manager, social workers, occupational therapists, social care assessors and a team co-ordinator.
As a team we work alongside a number of different cohorts; including people with a learning disability, people with long term conditions, older adults, people with needs around their mental wellbeing, individuals with physical disabilities and people who require support around their drug and alcohol use.
It is essential that workers have a robust knowledge of the Care Act 2014. Other important legislation includes the Mental Capacity Act 2005, Safeguarding Adults, and the Human Rights Act 1998.
The role requires a practitioner to be confident in their own assessment skills, and able to work independently and as part of a team.
We are seeking a person with a strong work ethic who strives to support others to remain independent through a strengths-based and person centred approach.
The work undertaken on the team can be fast paced and complex.
We work alongside families and carers.
We work closely with our partner agencies which may include GP surgeries, Urgent Community Response teams, community health staff, specialist health teams, The Royal Devon University Hospital, local housing and the voluntary sector.
You will receive regular 1:1 and peer supervision and the opportunity to develop your career in social care.
