We are seeking a highly motivated Patient Safety Investigator to join our Patient Safety Team, with additional responsibilities supporting inquests. The successful candidate will promote and advance the patient safety agenda across the Trust.
You will support the daily management of patient safety incident processes and oversee investigations of incidents within the Trust, in accordance with the Patient Safety Incident Response Framework (PSIRF).
The role involves leading and contributing to Trust training on patient safety initiatives. This position offers an opportunity to positively influence patient care, improve services, and enhance patient and carer experiences.
Applicants should have experience conducting investigations under PSIRF and demonstrate an understanding of the Systems Engineering Initiative for Patient Safety (SEIPS) framework. A passion for supporting patient safety and the patient experience is essential.
The Post Holder will:
1. Manage patient safety incidents in line with PSIRF.
2. Conduct initial response reviews to determine appropriate incident responses.
3. Ensure involvement of service users, families, staff, and third parties in response processes.
4. Coordinate investigations, case reviews, and learning events to foster learning from incidents.
5. Support and prepare witnesses for inquests, attend inquests, and record proceedings when required.
6. Deliver training on Patient Safety Incident Response and SEIPS to staff.
