Overview
We have an exciting opportunity for a Band 6 Occupational Therapist with mental health experience to join our Community Older Persons Mental Health Occupational Therapy Team. In this role, you will provide specialist Occupational Therapy assessment and intervention as part of an MDT, working with individuals in the community experiencing functional mental health conditions, cognitive impairment and Dementia. You will benefit from regular professional supervision with the OT Team Lead, as well as peer supervision and support through the Band 6 Professional Development Forum.
Responsibilities
* Promote recovery and self-management strategies to empower service users.
* Conduct comprehensive OT assessments to inform intervention plans, identify strengths, and develop adaptive strategies.
* Play a vital role in maintaining independence, promote self-management, reduce hospital admissions and supporting inclusion.
* Actively contribute to risk assessment, ensuring effective risk management in collaboration with the MDT.
* Provide an occupational perspective within MDT care plans.
