Job overview

The Cardiology Nurse Specialist role will work as a skilled, autonomous practitioner as part of the multidisciplinary team to deliver high quality specialist care for patients diagnosed with acute cardiology presentations throughout their care pathway.

1. Responsible for the day to day running of Nurse led Cardiology Clinics.
2. Be responsible for the assessment, planning, implementation and evaluation of care for cardiology patients within the Emergency Zone without supervision on a daily basis and to facilitate ‘virtual cardiology round’ in conjunction with a Consultant Cardiologist
3. Provide specialist advice to the multi-disciplinary team, facilitating early transfer to the Cath Lab/CCU
4. To co-ordinate the safe transfer of patients through the inpatient system to allow for sufficient capacity and placement of emergency admissions to CCU/Cardiology wards and/or complex care area.
5. To ensure that the highest standards of evidence-based patient centered care.
6. Work in partnership with the multi-disciplinary team.
7. Participate in the education programme within the emergency zone and cardiology departments
8. To participate in the monitoring, evaluation and auditing of services and to present these findings as part of the Clinical Governance framework

Main duties of the job

9. To provide specialist nursing advice and support to patients, their families and carers from the time of diagnosis and throughout treatment and follow-up phases of treatment and care.
10. Facilitate early provision of specialist cardiology expertise to in-patients admitted within the medical division to ensure equitable care irrespective of geographical location.
11. Work with Specialist Cardiology Consultants to develop and implement evidence-based care pathways to guide diagnosis and management of cardiac conditions according to local and national guidelines, irrespective of geographical area.
12. Support the development of nursing services for people with Cardiac conditions, working across organisational boundaries, ensuring opportunities for early supportive discharge to specialist community teams are maximised.
13. Be accountable for an individual and shared caseload of patients referred to the Cardiology Service in NBT

Detailed job description and main responsibilities

14. •Demonstrate specialist cardiology knowledge and skills relating to Cardiac patients across a range of procedures and practices.
15. •Act as a clinical resource in the management of people with Cardiology conditions for all healthcare professionals within NBT, Primary Care Trusts and the Cardiac Network.
16. •Manage an outpatient caseload, receiving referrals for patients with newly diagnosed cardiac conditions from both primary and secondary care and those with an established confirmed diagnosis.
17. •Prioritise caseload according to clinical stability of patients, taking considering the unpredictable nature of a caseload.
18. •Maintain standards and practice in accordance with the NMC Code of Professional Conduct, national guidelines, and local Trust policies/guidelines.

19. Registered Nurse with current NMC Registration

22. At least 3 years post grad experience with 2 years experience in acute cardiology
23. Experience of managing patients with Cardiology conditions

Desirable criteria

24. Physical assessment and clinical reasoning module or eqivalent
25. Audit and Research experience

The successful applicant(s) will normally commence at the minimum of the scale unless they have previous NHS service at the same band. Progression through the scale is by annual increments.
