Job overview

An exciting opportunity has arisen for a Critical Care Outreach Practitioner in the friendly Critical Care Outreach team at the Kent and Canterbury Hospital; they will be responsible for overseeing the high standards of care required for looking after the deteriorating/critically ill patients.


Main duties of the job

Main Duties can be located within the Role Job Description.

The post holder will work at an enhanced level of practice utilising highly developed post graduate experience, knowledge and skills in Critical Care specialty. They will manage discrete aspects of a patient’s care in collaboration with the multidisciplinary team, within their current level of practice and patient group, aligned to the four pillars of practice and aligned to Trust policy, ‘Enhanced, Advanced and Consultant Level Practice- Nurses, Midwives and Allied Health Professionals (AHPs).’

The post holder will contribute to nursing/midwifery/AHP practice to improve the quality of care to patients.

Skills and experience

* Significant postgraduate healthcare experience demonstrating professional development across the four pillars of practice.
* Significant experience of care in critical or emergency care medicine.
* Experience of working with patients/service users to resolve concerns and improve their experience.
* Evidence of effective networking
* Experience of supervising staff and delivering training
* Post graduate qualification in consultation and clinical assessment
* Experience of presenting to groups of staff/public


Personal/professional attributes

* Leading in quality improvement project within clinical speciality
* Evidence of delivering education and training to individuals or groups


Qualifications

* Current NMC/HCPC registration.
* Degree in nursing or working towards degree or equivalent
* Preperation of mentors or equivalent
* Evidence of continuing professional development within Critical Care or Emergency Care Practice.
* Master’s degree or working towards this qualification
* Advanced physical examination qualification
* Advanced practice or Masters/L7 programme commenced


Other

* Flexibility in working patterns to meet the needs of the service
* Awareness of and adherence to the organisations vision, mission and values
* Commitment to equity and diversity
* Self-motivated, positive, adaptable and flexible approach
* Knowledge of best practice, key issues and developments in the deteriorating patient programme


Governance

* Understanding of quality governance, patient safety and patient experience tools to support clinical practice and service improvement.
* Sound knowledge and understanding of national and local policy and guidance
* Utilisation of clinical effectiveness tools to deliver quality improvement e.g. risk management and clinical audit.
* Evidence of innovative practice utilising research and audit.
* Experience of contributing to quality improvement in safety/quality improvement projects.
* • Experience of writing reports and presenting to teams or small groups
* Experience of implementing and evaluating improvements in care
* Experience in audit and clinical data collection
