Qualified Social Worker - Adults Hospital Discharge Team (HiCAF) in Runcorn (UK). This role is full-time and located in Runcorn, supporting Halton Council's efforts at Whiston Hospital during the bustling winter period. What you will do:
* Work collaboratively with medical and social services teams to ensure safe and efficient hospital discharges
* Manage a complex caseload, demonstrating the ability to work independently and think on your feet
* Provide tailored support and intervention plans for individuals and families transitioning from hospital to community-based care
* Liaise effectively with multidisciplinary teams to ensure seamless and person-centred care
* Adapt quickly to a fast-paced environment and swiftly address the individual needs of patients
* Use your skills to mentor and guide less experienced staff members when needed
About You:
* Previous experience working within an Integrated Hospital Discharge Team is essential
* Familiarity with the Eclipse system would be advantageous
* Demonstrated ability to operate efficiently in a dynamic and high-pressure environment
* A full UK driving licence and access to a vehicle is required
* Strong interpersonal and communication skills to engage with diverse populations
* A degree in Social Work or related discipline, with a commitment to continuous professional development
