Role Overview
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver a range of quality care services to patients visited by the Community Response Hubs based at either Chester, Tarporley or Ellesmere Port. The team visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.
Key Responsibilities
As part of a friendly team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post involves Lone working. and requires travel across the Western Cheshire footprint, access to transport for work purposes is essential and MUST be demonstrated in your application.

Training & Onboarding
All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction/initial training requirements of the role. Whether you are just starting out in your career or looking to use your existing skills and experience in a new role, you'll find something to suit you at CWP.
Working Hours & Pay
The hours are 37.5 per week. Consisting of 4 x 9.5 hour split shifts, worked on a rota basis between 08.30 - 14.00 & 18.00 - 22.00 including Bank holidays and weekends. The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.
