An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to come and join us in our Community Response Team. Our Community Response Team covers South Warwickshire, has bases at Stratford upon Avon and Leamington Spa working across the county supporting patients that have identified health and social needs as a result of a change in their health condition. The service is committed to providing high quality care to provide an immediate response, aiming to prevent avoidable hospital admissions by providing support to patients to remain independent within their own home and facilitate safe discharges home from hospital. The team works together to support patients in the community and work closely with social services, GPs, community nursing teams, patients, relatives and carers.
To be successful in this role you must hold a valid driving licence and have access to your own car.
The Community Response Team operates across the hours of 08:30-21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days. The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate personal care and associated duties under the direction but not direct supervision of the registered staff and working as part of the multidisciplinary team.
As a Band 3 Clinical Support Worker (CSW) you make a valuable and important contribution to the delivery of high quality healthcare, supporting the registered professional in the delivery of safe, effective, compassionate patient care.
