Overview

Are you an experienced registered nurse looking for a rewarding role where you can make a significant impact on patient care pathways? We are seeking a dedicated and clinically confident Senior Band 6 Nurse Assessor to join our dynamic Continuing Healthcare (CHC) team. This is your opportunity to use your clinical expertise in a different way, leading on complex assessments and ensuring that individuals with profound health needs receive the fully funded care they are entitled to. You will be at the heart of decision-making, collaborating with a multi-disciplinary team to champion patient-centred outcomes.

The CHC Nurse Assessor will be responsible for conducting face-to-face Continuing Healthcare assessments for individuals across Northamptonshire. Key duties include facilitating multi-disciplinary reviews to determine eligibility under the National Framework, managing a complex caseload, and making autonomous, evidence-based decisions on patient needs. The role requires building strong relationships with clinicians, social care professionals, and families, while ensuring all recommendations and records comply with clinical guidelines and data protection standards. Regular travel within the region is essential for this patient-facing position.

Responsibilities
