Reports to: Clinical Lead Community Frailty Hub

Location: York Community Frailty Hub, Gateway 1, Holgate Park Drive, York, YO26 4GG, with a minimum of one day per week at the Scarborough Virtual Frailty Ward

Hours: Part-time or Full-time

Salary: £43,923 per annum

We are seeking to appoint doctors with an interest in frailty to join our innovative Community Frailty Hub.

This is an exciting opportunity for doctors who have completed Foundation Training and are looking to develop specialist experience in frailty, community-based medicine and integrated multidisciplinary care. Working alongside experienced GPs with Extended Roles (GPwERs) in Frailty and a highly skilled multidisciplinary team, you will play a key role in supporting older adults living with frailty across York and North Yorkshire.

The Community Frailty Hub brings together health, social care and voluntary sector partners to provide coordinated, proactive and person-centred care. As demand continues to grow, with the population aged over 85 projected to increase significantly over the next decade, this service is at the forefront of developing new ways of delivering care closer to home.

This role offers structured clinical supervision, dedicated frailty education and the opportunity to gain experience across multiple innovative services including Frailty Advice and Guidance, Urgent Community Response, Comprehensive Geriatric Assessment and Virtual Frailty Ward care.


Main duties of the job

The post is offered as a 12-year fixed–term contract with the potential for progression into a substantive role and a structured frailty specialty development pathway.

You will develop expertise in assessing and managing complex older adults in community settings, contribute to crisis response services that help patients remain safely at home, and work collaboratively with professionals across health, social care and the voluntary sector.

* Urgent Community Response (UCR)
* Virtual Frailty Ward care
* Multidisciplinary team decision-making and care planning
* Integrated working across primary, community, social care and voluntary sector services
* Quality improvement and service development initiatives

Job responsibilities

What You'll Do

* Provide clinical advice and support for frail and vulnerable patients living in the community
* Undertake community-based clinical assessments, including home visits where required
* Contribute to the Frailty Advice and Guidance (A&G) service
* Support delivery of Comprehensive Geriatric Assessments (CGA)
* Work within the Urgent Community Response (UCR) pathway to support safe patient care at home
* Participate in the Scarborough Virtual Frailty Ward under GPwER Frailty supervision
* Participate in multidisciplinary team meetings and collaborative care planning
* Initiate appropriate investigations, referrals and management plans
* Prescribe safely and appropriately in accordance with NICE guidance and local policies
* Maintain accurate and contemporaneous clinical records using SystmOne
* Work collaboratively with nursing, therapy, social care, voluntary sector and medical colleagues
* Participate in regular clinical supervision, education and professional development activities
* Support service improvement initiatives and pathway development

About You:

* AGMC registered doctor who has successfully completed Foundation Training (or equivalent)
* Passionate about delivering high-quality care for older adults living with frailty
* Interested in developing a career in frailty, geriatrics, palliative care, dementia services, community medicine or general practice
* An effective communicator who enjoys working collaboratively within multidisciplinary teams
* Motivated to learn and develop within an innovative and evolving service
* Comfortable working under supervision within a structured and supportive clinical environment


Person Specification


Qualifications

* GMC registered doctor
* Completed Foundation Training (or equivalent)


Experience

* Some experience in acute or community clinical assessment
* Experience in geriatric medicine or community care
* Evidence of CPD in older adult care


Skills and Abilities

* Strong communication and MDT working skills
* Ability to work under supervision within a structured service


Personal Attributes

* Interest in older peoples medicine / frailty
* Interest in developing a career in frailty, palliative care, dementia services or general practice
