Overview
An exciting opportunity has arisen to work as a NRFP Social Worker with the adult Response Team. We are seeking a dedicated and experienced Social Worker to join our team. This role involves working collaboratively with multidisciplinary teams to support individuals who are eligible for social care support. You will ensure that care is delivered effectively, with the client’s well-being at the heart of every decision.
Responsibilities
* Work within a multi‑disciplinary social work team and with the NRPF, Refugee and Migrant Service to meet the diverse needs of the community we serve.
* Work collaboratively with health and social care professionals to develop personalised care plans.
* Advocate for clients and their families, ensuring their voices are heard throughout the processes.
* Support decision‑making around funding eligibility and care planning.
* Ensure compliance with legal and regulatory frameworks, including the Care Act 2014 and Mental Capacity Act 2005.
* Maintain accurate and timely records of all interactions and decisions.
* Serve as a core member of a multi‑disciplinary team of social workers and case managers.
Qualifications
A degree in Social Work and registration with Social Work England (or equivalent body).
Demonstrated commitment to promoting person‑centred care and empowering individuals.
Ability to work collaboratively as part of a multi‑disciplinary team.
