Job Overview

Band 8A Senior Patient Safety Lead; Patient Safety Specialist (Permanent or Secondment/Job Share). Promote a safety and quality culture across Maternity, Gynaecology, Neonatal and Paediatric services, taking a lead in supporting local investigations and responses to incidents in Obstetrics and Gynaecology.

Responsibilities include leading the Maternity, Gynaecology, Neonatal and Paediatrics patient safety agenda and ensuring a unified approach to the review of patient safety incidents across the service, in line with Trust policies, procedures and best practice guidance.


Main Duties of the Job

* Ensure effective governance and risk systems across maternity, gynaecology, neonates and paediatrics. Work closely with Women and Children’s services management to ensure effective communication systems and compliance with national and local standards.
* Lead the Patient Safety Team within the PSIRF framework and cooperate with the Trust Patient Safety Team.
* Act as single point of contact for Maternity & Neonatal Safety Investigations (MNSI); coordinate documentation uploads and interview arrangements.
* Participate as a member of the Perinatal Mortality Reporting Tool Group; review all fetal losses and neonatal deaths and attend related meetings.
* Liaise with Professional Midwifery Advocates (PMA) to support midwifery staff involved in clinical incidents and investigations.


Detailed Job Description and Main Responsibilities

* Work with women’s services management and clinical governance leads to ensure effective governance arrangements and reporting.
* Manage all aspects of the clinical governance and risk portfolio within Women & Children’s Services (incidents, clinical effectiveness, audit, complaints).
* Assist in investigations of complaints as appropriate.
* Collaborate with medical, midwifery and nursing teams to implement changes identified through governance processes.
* Provide reports demonstrating compliance with CQC Outcomes for Quality and Safety and NHS Improvement Quality Governance Framework.
* Ensure all maternity incidents that may result in severe brain injury are reported to the Head of Legal Services within 48 hours.
* Ensure cases meeting the Each Baby Counts (RCOG) criteria and any maternal deaths are notified to MNSI for investigation within set time frames.
* Oversee tracking of progress against LMNS, ICB plans and transition milestones, flagging issues, risks and concerns to stakeholders.
* Provide reports for BOB LMNS regional committees, Quality Assurance and Learning Committee, and Patient Safety Committee.
* Support ward and department managers in risk assessment and management decisions.
* Manage the maternity, gynaecology, neonatal and paediatrics risk register.
* Analyse complex data from complaints, incidents and patient feedback; present findings to senior managers, consultants and colleagues.
* Represent the Trust at STP Local Maternity System governance meetings.
* Ensure compliance with Trust-wide policies and guidelines; disseminate safety alerts in a timely manner.
* Provide written monthly and quarterly reports on risk, incidents and complaints, with recommendations and progress updates.


Person Specification


Essential Criteria

* NMC registered Nurse or Midwife with significant midwifery experience.
* Leadership experience.
* Organisational change experience.
* Significant governance experience.
* PSIRF, patient safety training and governance experience.
* Emergency risk management training.


Desirable Criteria

* Registered midwife.
* Human factors training.
* Master’s degree.
* Risk management qualifications.
