Social Worker – Adults Learning Disabilities (Community Care) Location: London Borough of Merton Rate: £32 per hour Umbrella (£24 PAYE) Contract: Temporary (with strong extension potential) Hours: Monday – Friday, 9:00 – 17:00 Working Pattern: Hybrid (3 days office / 2 days home) Service Care Solutions are currently recruiting for a Social Worker to join the Adults Learning Disabilities Community Care Team within the London Borough of Merton. Key Responsibilities: Undertake the full Care Management process in line with the Care Act and statutory guidance Manage a caseload as allocated by the Team Manager, maintaining accurate and up-to-date records (paper and IT systems) Complete risk assessments in line with Merton Care Management Procedures Deliver duties under relevant legislation including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS) Support safeguarding enquiries and promote Making Safeguarding Personal Commission care packages, ensuring value for money and quality service delivery Maintain accurate records on CareFirst and other systems Attend meetings, court hearings, and tribunals as required Engage in joint working with partner agencies and stakeholders Contribute to service development, data reporting, and continuous improvementRequirements: Qualified Social Worker with relevant registration Experience working within Adult Social Care, ideally Learning Disabilities Knowledge of the Care Act, MCA, DoLS, and safeguarding processes Strong assessment, communication, and case management skills Ability to work both independently and as part of a teamThis is a great opportunity to join a supportive and forward-thinking local authority with flexible working arrangements.
