Main duties of the job
As a Specialist Rotational Pharmacist, you will:
* Collaborate on prescribing decisions with multidisciplinary teams.
* Undertake rotations in Oncology/Haematology, frailty and older person care, Urgent and Emergency Services, among others.
* Monitor prescriptions and develop patient-centered pharmaceutical care plans.
* Ensure seamless pharmaceutical care through accurate drug histories and discharge planning.
* Contribute to medicines safety and quality initiatives.
* Mentor and train pharmacy and healthcare professionals.
* Engage in education and health promotion activities.
* Represent pharmacy in Trust meetings.
* Participate in a weekday service with some weekend and on-call duties, initially home-based.

Experience
* Master of Pharmacy
* Current GPhC registration
* Diploma in pharmacy practice or equivalent
* Completed general level competency training
* Experience in pharmaceutical environment and hospital pharmacy
* Knowledge of medicines guidelines
* Specialist knowledge in clinical speciality
* Independent prescribing and specialist training in Clinical Speciality
