Community Services Pharmacist

Are you seeking a new challenge working in a patient-facing role in Community Services? We have a position available for an experienced pharmacist to join our established Community Services Pharmacy Team. We are a team of skilled, patient-focused clinical pharmacists and pharmacy technicians who provide a clinical service to community hospital adult inpatient units and a home-visiting medicines optimisation service to a referred caseload of patients with complex medication needs. Exmouth Community Hospital is the base site for this role.
Role responsibilities
* Provide medicines advice to patients, carers and health and social care professionals, and skilled non-registered staff to resolve complex medication queries.
* Medicines reconciliation on admission and discharge.
* Deliver medicines optimisation and personalised medication reviews as part of a wider multi-disciplinary team.
* Work with the pharmacy team to support safe and timely discharge of patients and manage a caseload of referred patients requiring interventions at home.
* Participate in shared electronic prescribing and medical record (EPIC) system use and contribute to integrated care.
Qualifications and experience
* Completed or on way to finishing a clinical diploma in pharmacy.
* Experience in a hospital pharmacy setting.
* Excellent communication, problem-solving, initiative, adaptability, organization and ability to prioritise workload effectively.
* Ability to work independently and as part of a team.
Working pattern
22.5 hours per week.
