We are recruiting for a Clinical Nurse Specialist to join our Locality Nursing Team on the Isle of Wight. Locality nursing is a community service that provides planned and unplanned care for patients in need. We seek a candidate who can independently perform advanced assessments of patients with diverse presentations and complex physical and psychological needs, supporting diagnosis and enabling the development and delivery of individualised treatment programs.
The role involves planning and managing complete episodes of care, undertaking independent comprehensive assessments, and managing service users with consideration of pharmacological factors. The role also requires making complex clinical decisions regarding patient management and outcomes.
Main duties of the job
1. Provide highly specialist, person-centred care that prioritizes safety, privacy, and dignity, using evidence-based clinical knowledge.
2. Promote community care delivered at home to prevent unnecessary hospital admissions and reduce inpatient stays.
3. Monitor indicators of long-term conditions, anticipate potential decline, and manage proactively to enhance well-being and independence.
4. Support the use of Telehealth to promote self-management.
5. Plan interventions based on needs, preferences, and wishes, involving family and significant others in discussions.
6. Collaborate with the In-Reach Service and develop communication with the Acute sector to support early discharge and patient flow.
7. Promote a positive image of community nursing.
8. Provide educational support to staff and patients.
9. Conduct holistic assessments of patient needs.
10. Liaise with General Practitioners, primary health care team members, and other agencies.
11. Offer information, support, and advice to patients, families, carers, and healthcare professionals.
12. Implement relevant policies and protocols.
13. Provide leadership to the multidisciplinary team when appropriate.
