The role
We are seeking Senior Social Care Assessors to provide community-based adult social care within Herefordshire. You will work mainly in city settings, applying your knowledge and experience in fast-paced teams. As a Senior Social Care Assessor, you will collaborate with occupational therapists, partners, social care assessors, and social workers to assess a diverse group of adults and informal carers. Your responsibilities include undertaking strengths-based assessments, mental capacity assessments, care management, and reviews. A driving license is required as travel is essential for this position.
About You
You will have:
1. An NVQ3 qualification or above in a related field, or equivalent experience. GCSE or equivalent in English and Maths.
2. Experience working in a care environment.
3. An understanding of current legislation and guidance affecting adult social care.
4. Knowledge of person-centred, strengths-based, outcome-focused assessment, care management, and review processes.
5. Experience supporting individuals to identify their desired outcomes and develop strengths-based plans to achieve them.
6. Ability to develop and maintain good relationships with service users, families, carers, colleagues, and external organizations.
