Main duties of the job
The post holder will
Support people with specialist palliative care needs to improve their quality of life within the community, ward, groups and onsite appointments.
Be registered with the HCPC and preferably have some experience of supporting people affected by life-limiting conditions.
Provide care for prevention and treatment of symptoms to enable optimal independence and participation in activities of daily living which are most important to each person we support.
Working alongside highly skilled practitioners, all experienced in providing this vital multi-professional approach within the hospice and the community.
Have working experience and skills in the management of respiratory, neurological, musculoskeletal, and oncological conditions.

We support 5 Clinical Commissioning Groups & have a robust governance structure in place. We provide inpatient clinical services within a modern, personal & friendly 18-bedded unit, as well as Specialist Community Crisis Support Service, Hospice at Home Service & Specialist Day Therapy Service. We also have the hospice Pepperell Education Centre that has an excellent reputation for delivering high quality education programmes and study days.

Job responsibilities
CLINICAL RESPONSIBILITIES
* To undertake specialist Occupational Therapy assessments in the ward, community and for onsite appointments to plan and prioritise interventions for individuals presenting with complex problems in the field of palliative care, maintaining records as an autonomous practitioner.
* To support and participate in the planning and on-going service development of the Occupational Therapy service, working with the Therapies Manager
* To promote the key values of specialist palliative care those we care for ensuring equitable access to the Occupational Therapy service.
* To act as a resource for palliative care Occupational Therapy and to provide advice, education and support to the other members of the Occupational Therapy team, wider multi-professional hospice team and other health care professionals.
* To keep up to date with developments in palliative care Occupational Therapy and maintain clinical practice and procedures in line with current best practice and the latest research outcomes, and monitor standards of care as outlined by the Therapies Manager.
* To contribute to local, regional and national networks & forums for specialist palliative care Occupational Therapy as indicated.
* To deputise for the Therapies Manager as required.
LIAISON
* Work proactively to maintain effective communication networks with all the
departments within the hospice to maintain a seamless delivery of
Occupational Therapy services
* Liaise with other professionals on behalf of the Occupational Therapy team
* Liaise with Primary Care teams to improve palliative care for patients in the
community
* Liaise with other services to ensure continuity of care
PARTICIPATION IN HOSPICE FUNCTIONING
* Act as an ambassador for the hospice at functions as required
* To participate in the life of the hospice
* To assist in tasks that affect the whole hospice, for example reviewing/designing new information booklets.
EDUCATION
* Act as a resource for members of other hospice teams
* Participate in the hospice in-house educational programmes and the in-service induction and training of other professionals and volunteers as required
* Participate in the hospice educational programmes for external participants as required
* To provide effective placements for Occupational Therapy students undertaking their professional training in conjunction with the Therapies Manager. To participate in the Mandatory Training programme.
QUALITY ASSURANCE
* Ensure the continued high quality service of the Occupational Therapy department by appropriate audit and standards
* Participate in research, audit and evaluation
* Support the Therapies Manager in quality management issues as they arise
