East Coast Community Healthcare CIC (ECCH) is an NHS community health and social care provider operating across the Suffolk/Norfolk border. The organisation is a social enterprise and staff-owned, offering NHS community health and social care services. This role is based at The Joseph Kittle Centre, Hemsby Road, Great Yarmouth, NR29 4QG.
The post is a [permanent] role with the following details: Band 5, pay £31,049 to £37,796 per year, working pattern: full-time or part-time. The service operates a 24/7 community nursing provision with core hours 08:00–16:30 and weekend/bank holiday working on a rotational basis. Applications from experienced staff and new registrants are welcome, with a Preceptorship programme available.
Job Description
Job responsibilities
* Contribute to, support, and promote ECCH’s strategic direction, values, and culture in relation to community health services.
* Discuss all treatment options with sensitivity and expertise, act as a patient advocate when appropriate, and maintain patient confidentiality with respect for diverse cultural backgrounds.
* Act as a resource for other health and social care professionals on community nursing matters.
* Participate in clinical supervision within Primary Care Home; support education and development in the clinical area.
* Develop internal and external networks to promote Primary Care Home.
* Contribute to clinical governance through clinical risk assessment and clinical audit.
* Foster an environment that supports high-quality clinical services and effective teamwork.
* Contribute to change and innovation within Primary Care Home and work as part of the multidisciplinary team to deliver planned and unplanned services.
* Collaborate on care pathways for diverse healthcare needs and review delegated caseload with senior Primary Care Home members to ensure high-quality care.
* Support patient self-care and independence using health coaching techniques; collaborate with residential care home colleagues to maintain high standards of care.
* Review policies/protocols/guidelines and develop clinical pathways and outcomes for proactive and reactive services.
* Plan and organise complex clinical activities across community settings to deliver best practice within your delegated area.
* Maintain personal and professional development; sustain competence and professional conduct in line with professional codes.
* Maintain legible, accurate patient records per ECCH policy and NMC standards; ensure compliance with Health and Safety policy, fire and environmental waste regulations.
* Adhere to ECCH policies and risk management systems; identify and manage risk within your area of responsibility.
* Manage cost-effective use of expensive clinical equipment and contribute to the efficient use of resources.
* Evaluate training programmes to support patients and carers in acquiring necessary knowledge and skills for independence.

Skills and Knowledge
Essential
* Excellent communication and interpersonal skills.
* Competent IT skills.

* Willingness and ability to work across different sites and travel as required.

Qualifications
Essential
* Professional Registration with Nursing and Midwifery Council (NMC) as Registered Nurse (Adult).
* BSc in Nursing Practice or equivalent.
Desirable
* Mentorship/preceptorship qualification (ENB 998) or evidence of working towards a nursing degree.
Experience
Essential
* Evidence of ongoing professional development.
* Ability to work unsupervised and demonstrate clinical knowledge within scope of practice.
* Previous experience in health care.
Desirable
* Recent clinical experience in community nursing and experience of multidisciplinary team working.
