About you

* Living in or within travelling distance of the BB postcode area?
* An effective, persuasive, and engaging communicator able to create a rapport in a face‑to‑face situation?
* A car driver with a full driving licence and your own vehicle? (Not essential if working door to door)

Bank Health Care Assistant – Inpatient Unit

Location: Blackburn


Key Performance Areas

* Under the direction of the clinical team/shift coordinator support patients and those that matter to them through the provision of personal care, clinical interventions and wellbeing and enabling support
* Deliver high standards of essential nursing care to patients and their families in keeping with care plans developed by registered practitioners
* Participate in the observation of patients and report changes in the patient’s condition to the registered nurse
* Support patients with therapeutic and diversional activity while promoting independence within patient limitations
* Work at all times to reduce risk, maximise quality of life and promote patients dignity and independence
* Provide support to families and those that matter to the patient as directed by the clinical team
* Communicate effectively with patients and relatives; be empathic and courteous, treating people with care and consideration at all times and relaying relevant information in a timely manner
* Ensure own entries in patient records are accurate and factual and maintained in line with hospice guidelines and standards of practice
* Recognise and respond to urgent situations, seeking assistance as necessary and relaying any relevant information on the condition of patients, their care or treatment in a timely manner
* Positively engage with opportunities for direct support and supervision, the development and maintenance of skills and knowledge
* Work across clinical areas during the span of duty where this is necessary to meet patient need or for own development needs including for example Supportive Care (Hospice At Home) and Support and Wellbeing
* Work effectively as a team member to provide excellent standards of patient care within the Inpatient Unit
* Be responsible for the assessment planning and evaluation of effective programmes of care for patients seeking support were needed
* Be responsible for delivery of care to individual patients within their team, ensuring compliance with legislation and best practice
* Be responsible for evaluating the care delivered in accordance with care plans and making relevant changes as
* Prioritise own workload and direct as appropriate the workload of others based upon patient need
* Ensure that patients and their carers receive the highest standards of care at all times
* Take responsibility for a shift as delegated
* Hold the keys to the Controlled Drug Cupboard when in charge being accountable for access to the Controlled Drug Cupboard
* Take responsibility for the 24‑hour health professional advice line working within established protocols as delegated
* Maintain clear, accurate documentation in accordance with best practice, professional standards and guidance
* Attend multi‑disciplinary meetings as appropriate within the unit/hospice
* Contribute to the discussions regarding patients they are caring for including liaising with healthcare professionals
* Provide opportunities to patients and relatives to discuss care and raise issues
* Responsible for complying with health and safety procedures
* Work as an effective team member offering support to colleagues as required
* Ensure confidentiality is maintained at all times and information relating to patients and personnel is only used in connection with authorised duties
* To work across departments where necessary in order to meet patient need.
