We provide safe, high quality care, treatment and interventions in patients’ usual place of residence and our @home service is at the forefront of healthcare delivery. Working as a key member of the multi-disciplinary team and with Community and Primary Care services across Lambeth and Southwark and St Thomas’ and King’s College Hospitals, you will help patients recover more quickly and comfortably, using advanced clinical skills and supervising and mentoring junior colleagues. We work with GPs, Hospital teams, Community Matrons, District Nurses, Therapy teams, and Social and Emergency care services. The service operates between to
7 days per week across shifts. You will able to work autonomously using advanced nursing practice and clinical reasoning and have skills and experience to bring to the team. We will offer you a wide range of opportunities to develop your clinical practice, holistic care skills and career.

Main duties of the job

To provide advanced clinical skills: assessment, diagnosis and treatment plans for acutely unwell patients at home (including care homes), to support advanced discharge from hospital and prevent unnecessary admissions. To lead, monitor and mentor assigned nurses, nursing and therapy assistants to deliver high quality, safe and effective care on a day-today basis. To work collaboratively and fully participate in the multi-disciplinary @Home service to maximise the contribution of each discipline, develop the team and meet needs of patients referred to the service. To provide specialised nursing advice relevant to the speciality. To liaise with District Nurses, Community Matrons, community services, GPs, the wider team, hospital teams, other professionals and voluntary agencies to facilitate admission, transfer of care and discharge which is patient and customer focused and reflects best practice. Participate in ‘case finding’ and the development of new ways of working and new care pathways.

Detailed job description and main responsibilities

·Independently manage a group of patients within the speciality whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care. Demonstrate appropriate practice and specialist advice in the assessment planning, implementation and evaluation of patient care and requesting of agreed diagnostic procedures/clinical investigations. Be responsible for the correct administration of prescribed medication including intravenous therapy. Be responsible for assessing and recognising emergencies in the speciality.To interpret information and take appropriate action and lead others to do the same. Act as an advocate for patients, to ensure a patient and customer orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.

Education & qualifications

Essential criteria

1. Registered Nurse on current NMC register.
2. Mentorship/Teaching qualification or equivalent experience.
3. Education to 1st degree level or equivalent. Evidence of relevant professional/clinical knowledge supplemented by specialist clinical, managerial training and CPD

5. Must have substantial post-registration experience in relevant specialty.
6. Ability to manage staff, conflict and difficult situations and to challenge and support others appropriately/assertively
7. Experience and competent in management of patients requiring acute intervention and monitoring Experience of direct supervision, teaching assessing others in a clinical setting Experience of identifying the development needs of others and giving effective feedback.
8. Experience of interagency and collaborative working.

Skills/knowledge/ ability

9. Ability to do advanced clinical assessments, plan, implement and evaluate specialist care delivered to patients and give advice
10. Demonstrates specialist expertise underpinned by theory acquired through CPD
11. In depth drugs knowledge and ability to administer prescribed medication and give IV therapy
12. Ability to analyse and interpret information and data and make quick and sound judgements and decisions.
13. Ability to work flexibly, prioritise and respond to changing demands in workload. And to work with uncertainty and changing situations
14. Ability to deal with sensitive information and challenging situations including end of life care
