The service delivers Urgent Community Response (UCR) and Virtual Ward services within the Hospital at Home Service. The base is in Ellesmere Port (Custom House) and the 7-day service operates from 08:00 to 20:00. The successful candidate will have a specialist interest in frailty and hospital admission avoidance to enable appropriate management of individuals in the community. The role involves working across 7 days, 8am to 8pm, as part of a holistic team including GP practices, Advanced Clinical Practitioners, Registered Nurses, Therapists, Therapy Assistants, Pharmacists and Clerical Support.
The service provides a 2-hour response to patients experiencing a health crisis to support treatment at home and avoid hospital admission. The successful candidate will also provide therapy assessment and treatment for patients on the virtual wards to optimise physical function. The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and Welsh patients, and works with wider Cheshire and Merseyside Health and Care Partnership. Locations include The Countess of Chester Hospital, Ellesmere Port Hospital and Tarporley War Memorial Hospital.
Responsibilities
* Provide a 2-hour response for patients entering a health crisis and ongoing assessments and treatment plans for individuals on the virtual wards.
* Work as an autonomous practitioner to perform specialist therapeutic assessment of patients with diverse presentations, using clinical reasoning to diagnose and deliver an individualised treatment programme in the patient\'s home.
* Collaborate with the Hospital at Home Team, Community Respiratory Team, Community Care Teams, Northwest Ambulance Service, Social Services, Millbrook and the patient\'s registered GP to support the patient at home.
* Work flexibly in response to service needs, ensuring allocation of an appropriate caseload for Therapy Assistants.
Qualifications / Requirements
* Specialist interest in frailty and hospital admission avoidance.
* Enthusiastic, flexible and innovative; able to work as part of a dynamic and pro-active team.
* Demonstrated ability to provide therapy assessment and treatment in community/virtual ward settings.
* Autonomous practitioner with clinical reasoning for diagnosis and treatment planning.
* DBS check paid by applicant with deductions from salary over the first three months; enrolment in the DBS Update Service encouraged (annual fee applies).
* New entrants to the NHS will commence on the minimum of the stated scale.
