Responsibilities
Provide a quality needs-led service for young people preparing to leave care and support them post-care.
Operate in a mobile flexible service requiring duties during normal working hours and evenings, with coverage arranged as needed.
Fulfill the role of a leaving care personal advisor for eligible, relevant, and former relevant young people as required by the Children (Leaving Care) Act 2000 and the Children Act 2010. This includes producing and reviewing the pathway plan and conducting regular two-monthly visits to care leavers.
Assist young people in contributing to the assessment of their needs prior to their transfer to the Leaving Care Team, engaging in reviews and discussions of the pathway plan for 16/17-year-olds with CLAFH colleagues.
Work within the Redbridge Leaving Care policies and procedures.
Maintain appropriate statutory and administrative records, including the use of IT systems, while observing confidentiality and data protection guidelines.
Assist young people and their social workers in preparing for leaving care by providing advice and a range of group work sessions focused on developing independent living skills, career or educational opportunities, and identifying accommodation options.
Provide support to young people post-18 through individual and group work sessions designed to improve life skills, relieve social isolation, and enhance peer group support.
Maintain knowledge of accommodation providers and work with young people to ensure contact and security of license or tenancy for accommodation is a priority, facilitating a smooth transition to permanent council accommodation.
Develop and maintain positive links with agencies such as Connexions, careers, training agencies, colleges, local employers, and the community to promote young people's access to employment, training, education, and leisure activities.
Involve young people in planning, delivery, and evaluation of services, encouraging their involvement in the Children in Care council and consultation systems in the borough.
Assess young people's financial needs, organize payment, and assist in claiming benefits or grants from education or charitable funds, supporting them in spending their leaving care grants if necessary.
Participate in staff training and development programs organized by the team or Department.
Ensure that equality and diversity considerations are central to your work, assessing the equalities impact of any major projects or initiatives.
Provide cover for the Practice Manager and represent them in their absence when required.
Maintain a smart and presentable appearance in compliance with the current dress code.
Perform any other duties appropriate to this area of work and consistent with the level of the post as required.
