You’ll work alongside people experiencing housing or wellbeing challenges, offering person led, trauma‑informed support that empowers them to create meaningful, sustainable change.
1. Your responsibilities will include:
2. Carrying out needs assessments to understand each person’s circumstances, strengths, and goals.
3. Co‑producing Individual Support Plans that promote independence and reflect personal aspirations
4. Reviewing support plans and risk assessments to ensure they remain relevant and empowering
5. Working collaboratively with local partners, housing providers, andmunity organisations
6. Offering advice and guidance throughmunity drop‑ins across Blaenau Gwent
7. Coordinating long‑term and crisis interventions, including attending joint meetings and case reviews
8. Supporting people to access information, digital tools, and resources to manage housing, benefits, and wellbeing
9. Providing resettlement support for those moving into new or independent amodation
10. Maintaining accurate case notes and sharing information appropriately with colleagues
11. Providing flexible outreach support in people’s homes and within themunity
What you’ll need to succeed:
12. Knowledge of trauma‑informed practice, attachment theory, and relationship based approaches
13. Experience supporting people experiencing emotional distress or working within mental health settings
14. Amitment to involving people with lived experience in shaping services
15. Excellentmunication skills
16. Understanding of adult safeguarding and the ability to maintain strong professional boundaries
17. Self‑awareness, reflective practice, and amitment to ongoing development
