Role Overview
As a Social Worker on our team, you will work as an autonomous practitioner within a multi‑disciplinary team, providing high quality social care services to adults experiencing mental ill‑health. Your core functions include assessment, care planning and coordination of care from a social perspective, and managing a caseload of adults with a wide range of mental health and social care needs.
Key Responsibilities
* Provide assessment, care planning, and coordination of care from a social perspective.
* Maintain a caseload of adults experiencing a range of mental health and social care needs.
* Liaise with care providers, acute inpatient units, the Crisis Team, Primary Care services, the Voluntary and Private Sector, and other agencies to coordinate an integrated recovery‑focused model of care.
* Ensure compliance with NHS Constitution values and raise concerns through appropriate line management processes.
* Deliver a compassionate, dignified and respectful service to patients at all times.
* Maintain accurate and timely records and participate in regular supervision.
* Provide leadership, supervision and support to junior staff, ensuring national targets and standards are achieved.
* Build trust and develop relationships to promote service user involvement.
* Carry out additional training, delegated responsibilities, and duties as identified by the line manager.
Qualifications & Experience
* Registered with Social Work England.
* Ideally one year of post‑qualifying experience in a relevant health or social care setting.
* For newly qualified social workers, demonstrated pre‑qualifying experience and values that benefit the team.
* Commitment to completing AMHP training, with support from the Trust.
