Overview
We are seeking a new Evening and Night nurse to join an established team with vacancies in both the North/South Nottinghamshire region. The role covers south Nottinghamshire (Rushcliffe/Nottingham North/East, Nottingham West), North Nottinghamshire (Mansfield/Ashfield/Newark/Sherwood) and Bassetlaw, working with Urgent Community Response (UCR) teams to ensure the right clinician attends urgent responses during the evening and night hours, maintaining safety and delivering nursing care when needed.
Our service is fast paced during out-of-hours periods with a 2–4-hour response time, covering nursing elements within UCR clinical conditions, including urgent end-of-life care, continence care, catheter care, and overnight wound care needs.
As the service transforms, we are looking for dynamic, autonomous practitioners with excellent communication skills who can be confident, organized, adaptable, reliable, and approachable. You’ll work independently in patients’ homes, collaborating with UCR services and the out-of-hours GP provider to ensure the right team attends the patient to meet urgent nursing needs.
Our team includes nurses and community support workers, working in partnership with community nursing day teams and UCR to provide a seamless nursing service from 6pm to 8am, putting the patient at the centre of their care and delivering effective patient care.
Responsibilities
* You will assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patient’s diverse needs, their carers and other professionals
* You will provide realistic, timely and occasionally intensive support to patients within a community setting
* You will provide education and supervision to junior staff members and other professionals
* Interventions will be evidence-based and designed to reduce stress and anxiety, manage risk and aid recovery
* You will maintain accurate, timely and relevant clinical records, both written and computerized, and ensure the safe custody of healthcare records
The post holder will need to be flexible and able to work across services as required.
