Job overview

Join the Merton Domiciliary Therapy team and play a pivotal role in delivering high-quality rehabilitation services to the local community. As an Occupational Therapist, you will be a key member of a multidisciplinary team (MDT), working collaboratively to support the rehabilitation of adults with frailty and complex health conditions. Your expertise will help patients optimise their independence, maintain their wellbeing and prevent unnecessary hospital admissions or premature moves to care homes.

In this role, you will assess and provide tailored treatment plans to patients in their homes, offering specialist support to enhance mobility and functional recovery. You will also contribute to the development of rehabilitation strategies and educate and supervise junior staff and students, ensuring the delivery of exceptional care. With a strong emphasis on fostering a culture of respect and support, you will help drive service improvements, providing clinical leadership and contributing to continuous professional development.


Main duties of the job

* Deliver high-quality, person-centred rehabilitation to patients in their homes, care homes, and community settings as part of the MDT.
* Assess and provide tailored OT interventions for patients with complex physical, cognitive and functional needs.
* Manage a caseload of patients, working autonomously and collaborating with MDT colleagues to ensure coordinated care.
* Promote independence through rehabilitation, mobility support, equipment provision and environmental adaptations.
* Supervise and mentor junior staff, students and rehabilitation support workers, fostering a supportive learning environment.
* Lead quality improvement initiatives and contribute to service development, audits and clinical pathway improvements.
* Liaise with statutory, voluntary and community services to support integrated care delivery.
* Maintain accurate clinical records and participate in governance activities to ensure high standards of care.
* Support the professional development of the team through training and continuous learning.


Person specification


Education/Qualification

* Degree or diploma in Occupational Therapy
* Current AHP with UK HCPC registration
* Up to date evidence & relevant knowledge through CPD as evidenced by portfolio
* Moving and Handling training
* Membership of Royal College of Occupational Therapy and / or relevant special interest groups
* Attendance at Clinical Educator Course


Experience

* Appropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community
* Appreciation of constraints to working in the community
* A clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit
* Ability to assess patient conditions, establish the patient’s problem, formulate and implement a treatment plan
* Has sound knowledge of the Mental Health Act, lone working policies, manual handling polices and other relevant legislations and policies
* Experience of MDT working and understanding their roles
* Some experience of supervising juniors/assistant/students
* Experience in community work within rehab
* Post registration experience at band 5 level in a wide range of clinical areas including elderly rehabilitation, community rehabilitation, falls management, dementia care, respiratory care and orthopaedics
* Experience of line managing / Supervision of staff
* Some understanding/experience in quality improvement and service evaluation.
* Aware of risk-management and issues relating to working the community and people’s home
* Experience in teaching and training
* Positioning and contracture management


Skills & Knowledge

* Excellent skills in the assessment and treatment of people with complex multi pathologies
* Ability to apply core OT skills across a range of clinical conditions within a patient centred, goal setting approach
* Knowledge of outcome measures and evidence-based practice associated with people with physical disabilities
* Ability to problem solve, prioritise and demonstrate advanced clinical reasoning
* Advanced communication and report writing skills, in both verbal and written format
* Skills in supervision and appraisal of assistants, junior therapists and students
* Presentation skills appropriate for teaching of junior staff, students and peers in in service training setting
* Knowledge and understanding of clinical governance
* An understanding of the importance of involving service users in their treatment and in service development
* Knowledge of project management including planning, implementation and review
* Involvement in quality improvement projects
* Highly developed specialist knowledge and application of assessments and interventions relevant to client group.
* Understanding and application of occupational therapy practice outcome measures
