City Health Federation is currently recruiting an experienced,enthusiastic, forward thinking Social Prescriber on behalf of BradfordCity 6 Primary Care Network (Horton PCN) - a network of 9 GP practices working collaboratively across the network area with our community partners tolook after the health and wellbeing of our local population. Social prescribing is a way of linking people tosources of support within the community. It provides health and social careprofessionals with non-medical / non-service focused options that can operatealongside existing treatments or care packages to improve health and wellbeing.Our aim is to support local people to live well and stay well, promote healthylifestyles and provide opportunities for socialization, skill sharing, healtheducation and emotional support.
Main duties of the job
We are looking for a highly motivated person who will work aspart of a multi-disciplinary team to provide expertise across the PCN.
You will be a central point of contact for patients, linkingthem with sources of support within local communities. You will take a person-centred approach towork with patients identified by GP surgeries and undertake guidedconversations with patients.
You will be expected to develop links with local GPpractices, colleagues within the service, local communities and VCS to ensuredelivery of integrated, patient focused care, demonstrating flexibility in therole, and a caring attitude towards patients.
This is an exciting position and you will needto be self-motivated and able to work on your own, whilst also integrating intoa number of well-established General Practices. You will have excellentorganisational, interpersonal and communication skills together with a goodworking knowledge of primary care and the challenges facing patient care in theNHS.
About us
Thisis an exciting opportunity to join this GP provider organisation encompassingGP practices across Bradford City with a total patient population of around160,000, and representing one voice for primary care in the City area. Youwill be expected to work across a number of Practices, with a set patientpopulation, alongside both the GP and nursing staff, to meet the needs ofpatients by providing direct care using established guidelines.
Further ongoing support is provided through bothlocal and countrywide governance and peer support groups. Additionalprofessional development in line with the individuals needs will be availablewith shadowing, e-learning, relevant courses/conferences and higher leveltraining opportunities being supported.
Job responsibilities
Receivingand actioning referrals from a wide range of agencies: GP practices,pharmacies, multi-disciplinary teams, hospital discharge teams, allied healthprofessionals, fire service, police, social care services, housingassociations, and voluntary organisations. (List not exhaustive).
Signpostingadults who have been identified as frail and socially isolated and others thatcould benefit from the Social Prescribing service, to social, recreational andnonmedical support within the community which may help them to improve theirhealth and wellbeing. You will be required to meet with patients for one-to-oneappointments to discuss local sources of support that may benefit them.
Workingwith adults with mild to moderate mental health, learning disabilities, anxietyand depression. Providing personalised support to individuals, their familiesand carers to enable them to take control of their wellbeing, liveindependently and improve their health outcomes. Develop trusting relationshipsby giving people time and focus on what matters to them, taking a holisticapproach, based on the persons priorities and the wider determinants of health.
Workingwith the patient to coproduce a simple personalised care and support plan which is tailored to theirpriorities, interests and motivations and is designed to overcome any barriersto success.
Maintainingexcellent community knowledge and undertaking research to identify the bestservices, groups, activities and other pursuits for a persons specific needsand goals.
Encouragingpatients to take independent steps to improve their wellbeing.
Facilitatingnew and innovative solutions to complex situations as well as common barriersfaced by people using the service.
Workingwith community organisations to identify local gaps, pressures and possiblesolutions. Effectively manage a variedcaseload of clients, ensuring that all quality and monitoring standards are metand that performance information is provided within the required timescales.
Assessingthe patients abilities and preferences, thinking laterally and then advisingand supporting them in a variety of areas/activities. Being sensitive tobarriers to adopting a healthier lifestyle such as affordability, accessibilityand life circumstances. Encouraging choices and actions that are acceptable andachievable to patients while being aware of cultural and social considerations.

Educatingnon-clinical and clinical staff within the Practice on other services that areavailable within the community and how and when patients can access them.

Maintainingcontact with the participants throughout the duration of the project, steppingup and stepping down support required as appropriate.
Recognisingthe importance of safeguarding procedures within the project and ensure anyconcerns, disclosures or allegations of abuse are immediately and correctlyreported.

Ensurethat all activities are monitored and evaluated.

Under supervision and support of GPs in practices, the post-holder willstrive to maintain quality within the practices, and will:
Participatein clinical governance activity and contribute to the improvement in quality ofhealth outcomes through audit, risk management and the Quality and OutcomesFramework.
Alertother team members to concerns about risk, quality and safety.
Participatein investigation of incidents and events as required.
Identify,apply and disseminate research findings relating to PCN practices.
Collectdata for audit purposes and use clinical audit to monitor quality in theservice and implement change according to findings.
Contributeto the effectiveness of the team by reflecting on own and team activities andmaking suggestions on ways to improve and enhance the teams performance.
Work effectivelywith individuals in other agencies to meet patients needs.
