We have an exciting opportunity for a Band 3 Community Health Care Support Worker to join our Community Learning Disability Nursing Team. The team strives to provide forward‑thinking, high‑quality services that work in partnership with other agencies. Post holders will have experience of working with people with a learning disability and enthusiasm to provide holistic person‑centred care. They will be able to communicate effectively with other staff, patients and their families/carers.
The role covers multiple bases across Derby City and Derbyshire, requiring travel to visit patients in the community. A minimum of NVQ 3 or equivalent is essential. Those working towards this qualification will also be considered.
Key Aspects of the Role
* Work as part of a multi‑disciplinary team to respond to nursing referrals and deliver person‑centred care.
* Implement individual care plans under the direction of the Lead or Registered Nurse, carrying out assigned tasks and interventions as required.
* Report any findings, changes or concerns regarding service users’ health, behaviour or environment promptly to the Registered Nurse.
* Contribute to risk assessments, treatment plans and team reports.
* Promote safety, dignity and wellbeing in line with Health & Safety policies.
* Manage challenging behaviour using approved methods and care plans.
* Travel to visit people in their homes and within the community, including areas not accessible by public transport.
* Maintain effective communication (verbal, non‑verbal, and written) with both service users and other professionals.
* Liaise with professionals, carers and agencies to ensure consistent and coordinated care.
* Share information and model good practice with carers and colleagues.
Responsibilities – Patient Care
* Carry out assigned tasks to meet the care needs of the people we support.
* Work under the direction of the registered professional to allocate patients following the plan of care.
* Contribute as part of the multi‑disciplinary team towards the implementation of care plans.
* Undertake a person‑centred approach to meeting the health needs of people with a learning disability.
* Work as part of the multi‑disciplinary team but will visit as a lone worker as appropriate, feeding back to the registered professional.
* Be aware of the risks, risk assessment and feedback any changes to the registered professional.
* Support registered professionals and the multi‑disciplinary team to collate information to inform their assessment.
* Be responsible for the designated work on a daily basis as agreed with the registered professional.
* Overcome barriers to communication using a range of communication methods, e.g., Makaton, and adapt information into a format that is accessible for people with a learning disability.
* Be aware of how to diffuse situations of aggressive and abusive behaviour guided by care plans, risk assessments, good practice and established guidelines, and report to the registered professional.
* Support access to health appointments following the nurse care plan in relation to desensitisation, reasonable adjustments and anxiety management.
* Encourage interest and engagement in a variety of daily activities, including self‑care, domestic skills, productivity and leisure.
* Establish and maintain good relationships with service users, carers both paid and unpaid and staff across multi‑agency boundaries.
* Have an awareness of, and be able to recognise safeguarding concerns and elevate these to a registered professional.
* Have knowledge of Positive Behaviour Support approaches.
* The post holder may be asked to undertake duties not directly highlighted in this job description. This will only be done when the skills and experience of the post holder meet the requirements of the role.

* Work without direct supervision but under the plan of care of the registered professional.
* Collaborate with all relevant parties to promote effective team working.
* Act as a role model and support new or less experienced staff and student nurses.
* Know when to escalated issues or concerns to the appropriate person.
