Social Care Assessor (Hospital Discharge Team) Location: Enfield Contract: Initial 3 months Hours: 36 hours per week Pay: £23.87 per hour (Umbrella) / £18.95 per hour (PAYE exclusive) About the Role Enfield Council is seeking an experienced Social Care Assessor to join our Hospital Discharge Team (HDT). This is a fast-paced, ward-based role supporting the safe, timely, and effective discharge of residents from hospital into appropriate care and support settings. You will work closely with NHS colleagues, families, and social care professionals to ensure smooth discharge pathways and positive outcomes for service users.
