Key Responsibilities
* Conduct comprehensive assessments of children, young people, and families in line with statutory frameworks and Cardiff Council procedures.
* Develop, implement, and review personalised care and support plans to meet individual needs.
* Provide guidance and support to families, carers, and professionals to enable effective care delivery.
* Collaborate with multi-agency partners, including health, education, and social care, to ensure integrated services.
* Safeguard the welfare of children and young people, promoting their safety and wellbeing at every stage.
* Maintain detailed and accurate records compliant with Council standards and legal requirements.
* Participate in professional supervision, team meetings, and ongoing professional development.
