The North East Kent Home Treatment Team is looking for a Registered Mental Health Nurse to join their team.
The community mental health nurse will carry a defined caseload of patients who have memory impairment and/or functional mental ill health.
The community mental health nurse is a key member of the multidisciplinary team and will have a continuing responsibility to assess, plan, deliver, and evaluate care, communicate findings, influence change, and promote health, best practice, patient safety, and experience.
The post holder must hold a full, valid UK driving licence and have access to a car/vehicle for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
1. Help manage a defined caseload of people with functional and organic mental illness under supervision.
2. Carry out comprehensive holistic assessments considering mental, physical, social, cultural, psychological, spiritual, and environmental factors in partnership with the patient and others through interaction, observation, and measurement.
3. Work collaboratively with other disciplines, patients, and their carers to develop a holistic, person-centred care plan addressing identified needs.
4. Deliver and evaluate safe, competent, person-centred care in partnership with service users and their carers to support recovery.

Person Specification
Qualifications
* Essential: A recognised professional qualification as a Registered Nurse (Mental Health) and NMC registration, or equivalent qualification for Occupational Therapy.
* Desirable: Practice Educator/Mentor Qualification
Knowledge and Experience
* Essential: Experience of acute mental health care and nursing a range of mental health conditions.
* Desirable: Experience of individual and group work
Skills
* The ability to travel across a large geographical area 24 hours a day.
* Knowledge of current best practices in providing holistic interventions for acute mental health episodes.
* Knowledge of relevant legislative frameworks and policies (e.g., Mental Capacity Act, Safeguarding, Consent to Treatment).
* A strong understanding of risk assessment and management principles.
* Understanding of the impact of acute mental health episodes on clients’ functions and occupations, as well as their families and carers.
