Main duties of the job
What You'll Do:
* Conduct holistic assessments addressing physical, psychological, and spiritual needs, creating personalised care plans for every patient.
* Deliver and continually evaluate specialist palliative care, ensuring interventions are evidence-based and tailored to each individual.
* Take charge of your shift as the Nurse in Charge, effectively allocating team resources and ensuring safe staffing levels.
* Support and guide families through open, sensitive communication, putting patients' wishes at the heart of their care.
* Stay on top of professional development and maintain up-to-date training and clinical competencies.
* Contribute to quality improvement through champion roles, audits, education, and policy work.
What You'll Need:
* Be a Registered Nurse with valid NMC registration and at least 6 months post-qualification experience.
* Be able to demonstrate the ability to work well within a team.
* Use initiative and have excellent communication skills.
* Have flexibility to work across days, nights, and weekend shifts.

Job responsibilities
* Undertake holistic assessment of patient needs, using appropriate assessment tools, recognizing physical, psychological, and spiritual needs, to develop personalized care plans.
* Provide evidence-based, personalized, specialist palliative care, including assessment, planning, implementation, and evaluation of care. Maintain accurate clinical observations and communicate effectively with patients and families.
* Coordinate and lead shifts as Nurse in Charge, allocating team resources and ensuring adequate staffing levels.
* Communicate effectively with family members, providing support and ensuring patient wishes are prioritized.
* Take on additional roles reflecting the evolving nursing standards, supporting service development.
* Engage in clinical supervision, professional development, and ensure compliance with mandatory training and revalidation.
* Participate in quality initiatives, incident reporting, and support student development as a practice supervisor or assessor.
* Coordinate patient discharge processes and ensure a safe environment, adhering to infection control and risk assessments.

Qualifications (Essential)
* Hold a Nursing degree.
