We are seeking a dedicated and skilled Advanced Practice Registered Nurse (APRN) to join our healthcare team. The APRN will play a vital role in providing high-quality patient care, conducting assessments, and developing treatment plans. Title: Advanced nurse practitioner in general practice

Salary Level Recommendation: Competitive rate agreed on experience

Hours of duty: to be agreed on appointment

Hanford GP started as a family ran practice and has grown to a well-established surgery with a list size of 7000 patients. 2 Partners are at the helm of the practice with 3 Salaried GPs and we take pride in being a training practice for GP Registrars.

The Practice has 2 large care homes along with a few smaller ones and the ANP will take a responsible part in the weekly ward rounds and care given to the residents.

As inevitable changes take place, we are wanting to meet the needs of the acute patient conditions with the skill of an independent prescribing ANP.

Job summary

The post holder will demonstrate to be a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and competent prescribing. They will communicate and work collaboratively with the general practice team, supporting the delivery of policy and procedures and a high standard of service of care to the patient.

Responsibilities

* Conduct comprehensive health assessments and physical examinations of patients.
* Develop, implement, and evaluate individualised care plans based on patient needs.
* Prescribe medications and manage treatment regimens in accordance with established protocols.
* Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
* Educate patients and their families about health conditions, treatment options, and preventive care.
* Maintain accurate and up-to-date patient records in compliance with legal and ethical standards.
* Participate in quality improvement initiatives to enhance patient care services.
* Stay current with advancements in nursing practice and healthcare regulations.

Requirements

* Must hold a valid registration as an Advanced Practice Registered Nurse with the Nursing and Midwifery Council (NMC).
* A Master's degree in Nursing or a related field is required.
* Previous experience in a clinical setting is preferred, particularly in primary care or specialty areas.
* Strong clinical assessment skills and the ability to make sound clinical decisions.
* Excellent communication skills, both verbal and written, with the ability to engage effectively with patients and their families.
* Ability to work independently as well as part of a multidisciplinary team.
* Knowledge of current healthcare policies, procedures, and best practices is essential.
