We are currentlyseeking a Lead Nurse who has an interest in Diabetes and would love to hearfrom you.

Our team is builton the principles of providing friendly, caring and compassionate healthcare toall and we are proud of our clinical and admin teams for their expertise,support and care.

We are activemembers of our Primary Care Network and are instrumental in the implementationof many PCN projects that are currently running.


Main duties of the job

We are seeking anexperienced and highly motivated registered nurse to join our team as our LeadNurse

Previous practicenurse experience is essential for this position

The ideal candidateshould have experience in chronic disease management (particularly Diabetes),QOF and Enhanced Service programmes, as well as general nursing duties,Managing a team and/or leadership skills.

Systm 1 (TPP)Clinical Software experience advantageous though not essential as full trainingwill be given.


About us

We are a GMSpractice with two Partners, 6 Salaried GPs, 2 Physician associates, 2 practicenurses, 2 HCAs. Supported by an experienced administrative team. We are also atraining practice with a GP Registrar and medical students. Practice list size7500.


Job responsibilities

To help develop thenursing services offered by the practice.

To provide clinicalleadership within the nursing team.

To ensure thehighest standards of care are provided for patients.

To manage anddevelop a clinical acute and chronic case load, dealing with presenting patientsneeds in a primary care setting.

Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care.

To deliver a highstandard of patient care using specialist practice Nurse skills Primary Dutiesand Areas of Responsibility Clinical Role.

Where necessaryevaluate patient test results in conjunction with the patient and their GP andensure the appropriate course of treatment is initiated.

Make appropriatereferrals to other members of the primary, community and secondary care teams.

Help the practicedevelop and deliver specialist primary nurse led services including chronicdisease management. Initiate the role of diagnosis for patients suspected tohave a chronic disease e.g. diabetes, COPD, asthma and CHD, referring to otherclinicians as appropriate.

Manage programmesof care for patients with chronic diseases by planning, providing andevaluating care under agreed guidelines and patient group directions. Maintaindisease registers in liaison with administrative staff. Undertake annual reviewof patients understanding and ability to self-manage.

Administer childand adult immunisations and vaccinations in accordance with national and localprogrammes. Competent in anaphylaxis and resuscitation techniques. Ensuringsafe storage, rotation and disposals of vaccines and drugs and that adequatestock levels are maintained.

Be able toundertake procedures including: venepuncture, glucose tolerance tests, BPmanagement, urinalysis, peak flow, spirometry, cytology and wound management.

Promote health andwellbeing, giving general education advice on diet, contraception, smokingchronic disease management, exercise etc.

Contribute to thepractice achieving its GMS contract obligations.

Maintain accurate recordsaccording to NMC rules.

Ensure all dataprotection requirements are met when gathering, recording and storing patientdata.

Liaise and maintaingood working relationships with all members of the practice team. Work withinthe multi-disciplinary team within the practice and across the wider healthsystem Teaching and Mentoring Role.

Promote a learningenvironment for patients, nurses and other health professionals Assist with theplanning and implementation and teaching for practice staff including medicalstudents, nurse students, nurses and health care assistants Act as mentor formore junior staff and students, assessing competence against set standards.

Undertake regularappraisals for the practice nursing team Professional Role.

Attend and contributeto relevant internal and external meetings as required including practiceclinical meetings, nurse team meetings and gold standard framework meetings.

Maintain a personalprogress and training plan to keep up-to date with current thinking anddevelopments as well as identifying own training deficiencies and attendingsuitable courses as appropriate.

Promote evidencebased practice through use of the latest research based guidelines.

Maintainprofessional registration and work within the latest NMC Code of ProfessionalConduct.

Record accurateconsultation data in patients records in accordance with NMC guidance and otherstandards and keep up to date with pertinent health policy and work with thepractice team to consider the impact and strategies for implementation.

Demonstrateleadership

Use technology andappropriate software packages as an aid to the planning, implementation andmonitoring of care, presenting and communicating information.

Work as aneffective and responsible team leader, supporting others and exploring themechanisms to develop new ways of working.

Prioritise own andothers workload and ensure effective time management strategies are embeddedwithin the culture of the team.

Manage the nurserotas/ holiday/ study leave.

* Awareness of blood-borne viruses
