Community Dementia Nurse Specialist

The post holder is accountable to the Operational Services Manager, will be clinically supervised by the area Team Manager, and undertakes duties without direct supervision. The role involves assessment and support throughout the diagnostic journey for individuals and their carers/family, working closely with the Older Adult Community Mental Health, Dementia & Frailty service, integrated community services, primary care, and other partners. The holder ensures integrated service provision, liaises with other services, and upholds EPUT’s ethos of good customer care.
Main duties of the job
- Act as an allocated senior nurse, undertaking Open Compassionate Empowering assessments and supporting individuals and their carers through the dementia diagnostic pathways.
- Maintain close links with the Older Adult Community Mental Health, Dementia & Frailty Service teams.
- Facilitate diagnosis by assessment and use appropriate individualized pathways.
- Fast‑track to dementia diagnosis, via suitable qualification and support, as identified.
- Offer expert prescribing advice on treatment of dementia and related psychological and behavioural aspects, and act as an independent prescriber if appropriately qualified.
- Ensure effective lines of communication and good networking with EPUT and other community, primary and social care services.
- Disseminate specialist knowledge across work areas, underpinned by sound theoretical knowledge.
- Participate with local MDTs when required.
- Responsible for monitoring the quality of service within the catchment area; participate in clinical quality assurance activities, lead audits, practice development, innovation, evidence‑based practice.
- In liaison with Associate Director and Team Manager, achieve agreed performance and activity targets, submit DDS and other required data on time.
- Review workload with Manager, ensure effective workload management and equitable distribution.
- Act as role model, demonstrate excellent leadership skills.
- Provide specialist advice, support, and consultation to staff across Healthcare teams.
- Conduct appraisals, meet training needs, adopt cascaded appraisal system.
- Establish and maintain professional and effective working relationships.

Qualifications and Education
- RMN Qualification
- Educated to Degree Level
- Additional Professional Qualifications (Tier 3) or Dementia Experience
- Identification of a Dementia Syndrome qualification or willingness to undertake training
- Non‑medical Independent Nurse Prescriber or willingness to undertake training
- Experience in the use of risk assessment tools
- Experience using screening tools in dementia
- Current driving licence

Salary
£49,387 to £56,515 a year, per annum + 5% High Cost Allowance.
