Job description
You will work as part of a multidisciplinary Early Supported Discharge (ESD) team for stroke and neurological rehabilitation, providing high‑quality rehabilitation in community and home settings and supporting patients to regain independence following hospital discharge.
* Deliver delegated rehabilitation programmes under the supervision of registered clinicians.
* Assist patients with functional rehabilitation tasks in their own homes and community settings.
* Help implement physiotherapy, occupational therapy, speech and language therapy, and nursing programmes.
* Undertake basic assessments and contribute to patient progress reviews under supervision.
* Promote independence, safety, and recovery following individual care plans.
* Support group‑based rehabilitation sessions and inpatient/community interventions where required.
* Serve as first point of contact for patients, families, and professionals when appropriate.
* Assist with service administration and maintain equipment as needed.
* Work flexibly across ESD and wider community neuro‑rehabilitation services as service needs require.
* Work a full‑time schedule of 8 am–4 pm, Monday to Saturday.
* Travel regularly to patients’ homes and between bases – a full UK driving licence and access to a vehicle are essential.
Qualifications
* NVQ Level 3 in Health and Social Care or equivalent qualification.
* Experience at Band 3 or similar rehabilitation/support worker role.
* Understanding of stroke, neurological conditions and rehabilitation principles.
* Experience working in community settings (desirable).
* Awareness of lone working in community healthcare environments.
* Understanding of confidentiality and information governance requirements.
* Ability to work independently, use initiative and follow clinical guidance.
* Good communication skills with patients, families and MDT colleagues.
* Follow delegated rehabilitation programmes accurately and safely.
* Willingness to undertake further training, including Trusted Assessor training.
* Full UK driving licence with access to a vehicle (essential).
