Complex Care Nurse Case Manager at Progressive Complex Care Provider
Lead & Transform Complex Care in Devon! Full-Time/Part-Time Options Available. £50,000-£55,000 per annum.

RoleRole & Responsibilities
You will conduct in-depth health assessments, develop and implement bespoke care plans, and coordinate care with a multi-disciplinary team. You'll empower patients and their families, monitor outcomes, and drive continuous improvement in care delivery. This leadership role offers the chance to make a tangible difference in complex care.
Main tasks include
* Conducting in-depth health assessments.
* Developing and implementing bespoke care plans.
* Coordinating care with GPs, therapists, and social workers.
* Ensuring the safety and effectiveness of care packages.
* Contributing to local business growth and commissioning initiatives.
* Empowering patients and their families.
* Monitoring outcomes and driving continuous improvement.
Requirements
* NMC Registered Nurse: Active PIN in Learning Disabilities Nursing or Children's Nursing.
* Proven experience in complex care or related nursing roles.
* Proactive, compassionate, highly organised, and a natural leader.
* Excellent communication and interpersonal skills.
* Strong analytical and problem-solving abilities.
* Ability to work independently and as part of a team.
* Commitment to providing high-quality patient care.
* Experience with care planning and coordination.
* Knowledge of relevant legislation and guidelines.
* Ability to manage time effectively and prioritize tasks.
