Job overview The North Lincolnshire Community Learning Disability Team is a specialist Health Care team working to meet the health care needs of adults with learning disabilities. We are looking to recruit a motivated and enthusiastic practitioner into this challenging and rewarding post. You will be responsible for assessing and supporting the health needs of adults with a learning disability under the supervision and guidance of the Senior Community Nurse. Advert The successful candidate will be responsible for a defined caseload of people requiring support from the service Completing a variety of assessments to determine input required and implementing interventions based on the outcome of these. Monitoring and reviewing the effectiveness of interventions and liaising with other members of the multi disciplinary team Working with individuals, families, other professionals and carers to ensure a holistic approach The ability to work as part of an established team and also have the skill and confidence to work autonomously when required.
